COPD survey

As part of the Cheshire and Merseyside pulmonary rehabilitation pathway improvement programme, the Innovation Agency have developed a survey to capture the thoughts and experiences of COPD patients regarding the support they receive for their condition.

Your local NHS wants to improve how you are supported to manage your COPD and are collecting feedback from patients and carers about the support you receive for COPD in your area. Your views and experiences will be used to improve the care you receive from your local NHS. We would be grateful if you can share your thoughts and experiences.
